The application form can be had form Room No. 163 at least one day before interview i.e. 2.30 pm after payment of Rs. 5/-. Complete applications will be accepted after registration between 10.00 am to 12.30 pm at Room No. 105, Office of Addl. M.S. (A) as per shedule, along with the enclosed attested copies of the DOB certificate/ Higher Secondary Education Certificate issued by the state board/ Caste Certificate/ Educational Qualification Certificate/ Emp. Exp. card (if available)/ Internship Completion Certificate/ DMC Regn/ Certificate of undertaking as the case amy be/ attempt certificate / latest photograph etc. in given sequence.
